Why home visits suit older adults
Getting to a clinic can mean organising transport, a long wait, and a tiring trip that undoes some of the benefit. A home visit removes all of that. More importantly, it lets the clinician see the real environment. The step at the back door, the height of the bed, the slippers worn in the kitchen. These details decide whether someone stays independent, and they are only visible at home.
What we help with
Strength and stamina. Muscle loss with age is normal but it is not fixed. A simple program using a chair, a bench and body weight can rebuild the strength needed for everyday tasks, and we progress it as you improve.
Balance and falls. Falls are the biggest threat to living at home. Our falls prevention program combines balance and strength exercises with a practical look at footwear and hazards. It is the most requested service we offer to older adults.
Walking and mobility aids. Advice on whether a stick, a frame or a wheeled walker is the right fit, how to use it well, and when to move on from it.
Pain. Arthritis, back pain and old injuries that make movement harder than it needs to be. We use hands-on treatment and movement to make daily tasks easier.
Feet. Nail care, skin care, diabetic foot checks and footwear advice from a podiatrist, at home, on a regular schedule.
After hospital. The first weeks home after an illness, a fall or an operation are when people are most at risk of going back. A short block of visits in that window makes a real difference.
How funding works
Government-funded aged care at home has changed over recent years, and the names of programs and levels can be confusing. The practical point is simple. If you have a care manager or provider, they can usually approve allied health visits as part of your plan and we invoice them. If you are waiting for funding or prefer not to use it, private visits are available, and a Medicare care plan from your GP may help with part of the cost. Our FAQ covers the details.
Working with your care manager
We send your provider the information they need, keep clinical notes, and provide short progress summaries so your care plan stays current. If your package is managed by a family member, we work with them in the same way.
Part of the Head2Toe group
Head2Toe Home is the home-visit arm of the Head2Toe group, with clinics in Moonee Ponds, Brunswick and Kew East. The clinician who visits is part of that team. If a clinic appointment ever becomes the better option, your records come with you.
