For participants, families and support coordinators

NDIS physiotherapy and podiatry home visits

Therapy that happens where you live, built around your NDIS goals. We work with participants, families, plan managers and support coordinators to make the most of your plan.

Functional assessments

A clear picture of what you can do now and what is getting in the way, written in the language your plan uses.

Therapy plans tied to your goals

Every program links back to a goal in your plan, whether that is getting to the letterbox, managing pain or building strength for work.

Reports for plan reviews

Progress reports and recommendations in the format the NDIA expects, delivered on time for reviews.

Equipment and home advice

Recommendations on assistive technology and small home changes, with the reasoning spelled out for your planner.

Podiatry for participants

Foot and nail care, footwear advice and orthotics assessment for participants who need it as part of daily living.

Carers and support workers included

We show support workers and family members how to help with exercises safely between visits.

Therapy that fits the plan, and the person

An NDIS plan is a set of goals with funding attached. Our job is to turn the allied health part of that funding into real progress. That starts with listening, because the goal written in a plan and the goal that matters most to you on a Tuesday morning are not always the same thing.

Home visits are often the most sensible setting for NDIS physiotherapy and podiatry. The barriers to daily living are at home, so that is where the assessment is most accurate and the practice is most useful. Transport is no longer a hurdle, and support workers or family members can join the session and learn the program.

Who we work with

Adult participants with physical disability, neurological conditions such as stroke, multiple sclerosis or Parkinson’s disease, chronic pain, and the mobility and foot care needs that come with them. We also work with participants whose primary disability is intellectual or psychosocial but who have physical goals in their plan. Children are seen at our clinics rather than at home, so for a child, please contact a Head2Toe clinic directly.

How we work with your team

We keep plan managers, support coordinators and families in the loop. That means a service agreement that is plain and short, invoices that match the plan line items, progress notes when you ask for them, and reports ahead of plan reviews rather than after. If a support worker will be helping with exercises, we write the program for them as well as for you.

What happens first

  1. You or someone on your team sends an enquiry with a few details and your funding arrangement.
  2. We call to talk through your goals and confirm what your plan covers.
  3. We send a service agreement for you to approve.
  4. Your clinician visits for a full assessment and sets up the program.

Registration and pricing

We charge at or below the NDIS price limits for physiotherapy and podiatry, including travel where it applies. If you need to know whether Head2Toe Home is an NDIS registered provider for a particular support, ask when you enquire and we will give you a straight answer.

Common questions

Which part of my plan pays for physiotherapy and podiatry?

Physiotherapy and podiatry usually sit under Capacity Building, in the Improved Daily Living category. Check your plan or ask your support coordinator. We can help you work it out before the first visit.

Can I use Head2Toe Home if my plan is self-managed or plan-managed?

Yes. We invoice you or your plan manager directly at the rates set out in the NDIS Pricing Arrangements and Price Limits.

What if my plan is agency-managed?

Agency-managed plans can only use NDIS registered providers. Send an enquiry and we will confirm whether that applies to Head2Toe Home for the service you need.

Do you charge for travel?

The NDIS allows providers to claim travel time for home visits within set limits. We explain what applies to your visits before anything is booked, so there are no surprises on the invoice.
